Registration for Medical Monitoring is Closed

Registration for the Medical Monitoring Program is closed.

The registration period began on January 22, 2018, and ended on October 22, 2018. Registrations are no longer accepted after October 22, 2018.

Entry Criteria

Your eligibility for the Medical Monitoring Program is subject to the Entry Criteria, as approved by the Court.  The Settlement Agreement Section D.5 specifies that “[a]ll class members must present reasonable proof of meeting the Entry Criteria” and that “[t]he Entry Criteria shall have three (3) components.”

1. First, each registrant must affirm, under oath, that he or she meets the Class Definition.
2. Second, each registrant must submit documentation that he/she resided in the State of Missouri on May 25, 2010.  Satisfactory documentation shall include and be limited to:  a) state or federal tax filing forms for the year 2010 reflecting a Missouri address; or b) a real estate or personal property tax receipt for the year 2010 reflecting a Missouri address or c) other government documentation acceptable to the Administrator.
3. Third, each registrant must substantially complete the Questionnaire [in the form to be agreed upon by the Administrator, the Hospital, Class Counsel and Defendants’ Counsel].

If your submission meets the Entry Criteria, and you registered before the Registration Period ended on October 22, 2018, you will be eligible to participate.

If your submission does not meet the Entry Criteria or you did not register before the Registration Period ended on October 22, 2018, you will not be eligible to participate.
